4. Florida Patient Rights & Responsibilities

In accordance with Florida Statute 381.026, patients have specific rights regarding their care. You may view the full Florida Patient Bill of Rights and Responsibilities at the official Florida Health website or request a copy from our front desk.

5. Medical Records & Financial Responsibility

  • Records Requests: Per Florida Administrative Code 64B8-10.003, Brain Optimization Institute may charge a fee for duplicating medical records, not to exceed $1.00 per page for the first 25 pages and $0.25 thereafter, plus actual postage.

  • Financial Responsibility: You acknowledge that you are financially responsible for all charges, regardless of insurance coverage. Brain Optimization Institute does not guarantee that functional neurology or regenerative therapies (such as HBOT or SoftWave) will be covered by your insurance provider.

  • Carrier Liability: Mobile carriers are not liable for delayed or undelivered messages.

7. Telehealth Services

For Florida residents, telehealth services are provided in accordance with Florida Statute 456.47. If accessing services from outside Florida, you acknowledge these sessions are for "Educational Coaching and Brain Optimization Consulting" and do not constitute a local medical diagnosis in your home jurisdiction.

8. Governing Law

These terms are governed by the laws of the State of Florida. Any disputes shall be resolved in the courts of Jacksonville, Duval County, FL.
